Abstract (original English)

Background Traditional fat grafting techniques predominantly rely on static assessment and homogeneous fat filling, yet clinical practice has revealed limitations such as poor adaptability to dynamic expressions and unnatural contour transitions. Recent studies have demonstrated that centrifugally stratified fat layers exhibit distinct biological properties. However, achieving harmonious integration of volumetric restoration and dynamic expression through precise dynamic evaluation combined with layered and progressive grafting of multimorphological fat remains a critical clinical challenge. Building on facial dynamized anatomy, this study proposes a dynamic assessment-multimorphological fat grafting strategy, aiming to establish a personalized rejuvenation protocol aligned with facial biomechanical principles. Methods A total of 305 patients (19-56 y old) were enrolled from December 2019 to February 2023. Fat harvested from the thighs or abdomen was processed into 3 forms: high-density macrofat, high-density microfat, and stromal vascular fraction gel. These were injected dynamically into multiple facial regions, angles, depths, and layers using specialized cannulas.
